This module is an overview of the first focus area of the equity-informed high-reliability learning series focused on advancing equity through five quality and safety focus areas. The main goals are to integrate equity into harm event reporting and associated high-reliability technologies and processes. Overall, this module seeks to engage participants in examples of how structural racism and other forms of discrimination contribute to poor quality care and suboptimal outcomes for historically disadvantaged individuals and communities; and offers high-level strategies for health system work that operationalizes equitable quality and safety practices to drive longer-term impact, accountability, and sustainability.
