The landscape of health care services delivery and community health is rapidly transforming. New competencies are required for clinicians and administrators to effectively navigate and succeed under emerging value-based payment models, population health performance standards and public health priorities.

This module acquaints learners with key and emerging Public Health, Population Health, Community Health and Health Equity concepts and frameworks related to maintaining the health and wellness of defined populations and communities. The module examines the importance of “upstream” social drivers of health at three levels - individual social needs, community-level social determinants of health, and broader structural determinants of health inequity. Learners understand how to apply these concepts to evolving care management, population health management, community health improvement, and public health priorities.

This is module one of the six-part Upstreamist Core Competencies Series.

In support of improving patient care, this activity is planned and implemented by The National Center Office of Interprofessional Continuing Professional Development (National Center OICPD) and HealthBegins. The National Center OICPD is accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

Organizations, not individual courses, are approved under this program. The National Center OICPD maintains responsibility for this course. Social workers completing this course receive continuing education credits.

The National Center OICPD (JA#: 4008105) is approved by the Board of Certification, Inc. to provide continuing education to Athletic Trainers (ATs). This activity was planned by and for the healthcare team, and learners who request it will receive Interprofessional Continuing Education (IPCE) credit for learning and change.

Physicians: This activity will be designated for 0.5 CME AMA PRA Category 1 Credit(s)™ through ACCME.

Nurses: This activity will be designated for .5 CNE nursing contact hours through ANCC.

Pharmacists and Pharmacy Technicians: This activity will be designated for .5 CPE contact hours (.05 CEUs) through ACPE.

Social Workers: This activity will be designated for social work continuing education credits through ASWB.

Athletic Trainers: The National Center OICPD (JA#: 4008105) is approved by the Board of Certification, Inc. to provide continuing education to Athletic Trainers (ATs). This program is eligible for a maximum of .5 Category A hours/CEUs. ATs should claim only those hours actually spent in the educational program.

Other health professionals: This activity was planned by and for the healthcare team, and learners who request it will receive .5 Interprofessional Continuing Education (IPCE) credit for learning and change.
