Dynamic Tremor in a Patient With Parkinson Disease After Deep Brain Stimulation

A 52-year-old man with Parkinson disease had a deep brain stimulation (DBS) system implanted 4 years prior for tremor management with good symptomatic control until 1 month prior to presentation, when he noted the onset of resting tremor in his left upper limb when abducting his right arm (segment 1). The tremor subsided with adduction. The tremor also appeared when he moved the implantable pulse generator (IPG) (segment 2).

A faulty connection between the IPG and its connection cable was identified, and the patient showed immediate improvement with a new IPG unit.
